Observation: ice-machine had build-up of mould/mildew along the ice-tray and hinges of the chamber. Correction: Stop the use of the ice-machine until the equipment is thoroughly cleaned and sanitize. Empty out the ice-machine and thoroughly clean and sanitize the chamber and all other components as per the manufacturer's instructions. In future, increase the frequency of cleaning the ice-machine.
