1)inside ice machine as there is mildew build up,
2)shelves and dishes in the storage room, lots of debris and dishes are dirty,
3) remove fly strip from storage room, fly strips cannot be located where food or utensils are being stored or prepared by Wednesday Nov 2

-all temperatures met health requirements
-temperatures are not being logged daily, log sheets were provided last time but are not being used
-Bleach bucket in kitchen now has bleach in it, due to staff allergy it was originally only soap and water
-pump dispenser required for hand soap for allergy to perfumed soap
-bleach at 100ppm in front hand sink
-dishwasher at 50ppm
-glasswasher at 12.5ppm Iodine
Evidence of smoking in the back storage room with the walk in coolers, cigarette butts in bucket of grease
