3) Hole under slushy machine in cupboard in bar area; holes observed in wall and ceiling of electrical room
4) Holes at bottom of drywall in washing machine area
5) Lack of door sweep/skirt under doors of electrical and hot water tank room

Corrective Action(s): 1) Repair/replace bottom of door sweep to walk in cooler.
2) Discuss with Pest Technician and determine if holes in grates are large enough for rodents to gain entry.
3) Repair holes. Seal with non-chewable materials
4) Determine if gap exists. Currently space to tight for proper assessment of gap by Health Inspector
5) Install door sweeps where gaps exist under doors of areas with high rodent activity

- under dishwasher (floor)
- in walk in cooler unit (floor)
- in corner of pizza cutting area (floor)
- Upstairs on wooden shelf in plastic knife box- items discarded during inspection

Corrective Action(s): Any remaining areas not cleaned and sanitized during inspection is to be cleaned and sanitized (with sanitizer solution) IMMEDIATELY.

- Proper thawing techniques observed
- Ice machine and scoop in good sanitary condition
- First In First out and date stamping methods observed in use
- Raw meat stored away from Ready to Eat food items
- Meat slicer stored in good sanitary condition
- Vegetable chopper stored in good sanitary condition
- Hair nets and hats in use
- Light covers in place
