Introduction

An unscheduled routine inspection was completed to assess compliance with the Community Care and Assisted Living Act (CCALA) and the Residential Care Regulation (RCR) and the relevant Director of Licensing Standards of Practice (DOLSOP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records, and information provided by the facility staff at the time of inspection.

Care systems reviewed during inspections include: Licensing, Physical Facility, Staffing, Policies and Procedures, Care and Supervision, Hygiene and Communicable Disease Control, Medication, Nutrition and Food Services, Program, Records & Reporting.

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Observed Violations
CARE AND/OR SUPERVISION: 34750 - RCR s.81(4)(b)(i) - A licensee must ensure that (b) each care plan is reviewed and, if necessary, modified (i) if there is a substantial change in the circumstances of the person in care.
Observation (CORRECTED DURING INSPECTION): One PIC had a change in transfer status, that had not been updated in the care plan. This was corrected by day 2 of the inspection.
Corrective Action(s): Ensure that each care plan is reviewed and if necessary, modified if there is a substantial change in circumstance of the person in care.

NUTRITION AND FOOD SERVICES: 37040 - RCR s.62(2)(b) - A licensee must ensure that each menu provides (b) for each day, at least 2 nutritious snacks, with each snack containing at least 2 food groups as described in Canada's Food Guide.
Observation: It was observed on 2 menus that at least twice, only one food group was offered during a snack (ie) fruit salad.
Corrective Action(s): Ensure that for each day at least 2 nutritious snacks are offered, with each snack containing at least 2 food groups.

NUTRITION AND FOOD SERVICES: 37190 - RCR s.66(1) - A licensee must ensure that each person in care receives adequate food to meet their personal nutritional needs, based on Canada's Food Guide and the person in care's nutrition plan.
Observation: Review of the facility's menus and corresponding menu audits, found that the minimum number of servings of fruit/ vegetables, and grain, did not meet the minimum requirements for at least one person in care.
Corrective Action(s): Ensure that the minimum number of servings is met for each person in care, based on their age and gender.

RECORDS AND REPORTING: 39320 - RCR s.83(4)(a) - Subject to subsection 83(5), a licensee, other than a licensee who provides a type of care described as Hospice, must (a) ensure that each person in care is weighed at least once each month.
Observation: For one person in care, it was described by staff that they are unable to weigh the PIC, and the person is not being weighed each month.
Corrective Action(s): Please ensure there is a plan to ensure a PIC is being weighed at least once each month.

Comments

Licensing officer was on site and the H.O.M.E society head office on August 15th, 2023 to review staffing requirements, any findings will be included on this inspection report.
LO provided a bulletin regarding the Nutrition and food section of the Residential Care Regulation. Also discussed during inspection: the meals and more manual.
It is requested that a written response be submitted on or before September 1st, 2023 describing how the above noted contraventions have been appropriately addressed and/or the plan for compliance with legislated requirements. The plan shall include a time line for any items that have not already been addressed. Please note that a follow-up inspection may be conducted to confirm compliance after the written response has been received by Licensing.
