Observation: There's a piping issue with the Men's washroom that the strata is working to correct. There's a large, gaping hole in the floor of the Men's washroom. This washroom is closed to the public until the issue can be fixed.
Corrective Action(s): Ensure the hole is kept covered when work is not being done in the washroom. Seal any holes around plumbing. The women's washroom is being used as a unisex washroom.

Observation: Mice droppings noted on the bottom shelf of hotholding unit, behind and around the ovens, and along the shelves beside the oven. Poison bait also noted on the shelves. Gap noted under the front and back door.
Corrective Action(s): As discussed, poison bait CANNOT be used inside restaurants. Stop this practice immediately. Clean all droppings using a bleach solution. Have the pest control company coming in twice a month until you have the pest issue under control. Seal the entry points under both doors.

502 - In operator’s absence, no staff on duty has FOODSAFE Level 1 or equivalent [s. 10(2)]
Observation: Neither of the two staff working had Foodsafe level 1 or equivalent certification.
Corrective Action(s): At least one staff member must be certified. This was mentioned in the previous report as well. You have until April 30, 2024 to meet this requirement.
