Observation: Sanitizer bucket (bleach bucket for cloths) has not been changed for a few day according to the cook.
Corrective Action(s): Bleach buckets must be TESTED for sanitizer throughout the day and the buckets changed, the cloths thoroughly rinsed, and a new bucket with fresh sanitizer used instead.

Observation: Dishes/pots/frypans double stacked in rack in dishwasher.
Corrective Action(s): Dishware and utensils must not be stacked in a dishwasher, only ONE LAYER of dishes can be processed at a time; dishware must be placed to allow the free flow of water around and along all surfaces of all dishware - DO NOT OVERFILL.

Observation: Numerous foods in the chest freezer, the stand-up freezer, the walk-in cooler and the dry storage area are not labeled as to contents, where purchased/processed/prepared and do not have either nutritional labels or ingredient lists.
Corrective Action(s): ALL foods must be labeled at to what it is, WHERE it was purchased, manufactured, processed, prepared, etc, WHEN it was purchased, manufactured processed, prepared, etc OR when it needs to be used by.
All meats must come from approved sources that are licensed to provide the product/s being ordered. All receipts must be kept on-site and available upon request.

Observation: Foods in freezers and walk-in cooler are being stored in non food grade plastic grocery bags that are in some cases torn so that cross contamination of food is occurring.
Corrective Action(s): All potentially contaminated food to be discarded and all other food to be placed in FOOD GRADE containers for storage, whether this storage time is short or longer duration (whether food is intended to be kept in the refrigerator or the freezers).
