Comments

Follow up inspection:
-Rodent droppings were not observed at the time of inspection.
-Operator had a flashlight on-site to check for rodent droppings.
-Two holes (one in the corner of the dry storage room and one behind the hot water tank) were sealed with steel wool. *Recommended: Cover the holes a metal screen as more of a permanent solution.
-A table was added in the back storage area to store rice higher off the floor.
-All dry goods were in pest proof containers with tight fitting lids.
-Take out containers in the sushi bar and utensils were covered.
-Screen-door at the back was in use and front door was closed.
-Operator mentioned that they would send copies of the pest control reports each month to district Environmental Health Officer (email address is on business card).

Please continue monitoring for signs of recent pest activity (rodent droppings, holes, gnaw marks, live/dead mice) and continue with pest control measures discussed and documented on the routine inspection report. Inspection report was translated to the operator by a staff member.
