Observation (CORRECTED DURING INSPECTION): Sliced mushrooms stored at room temperature since about 1130am -time is now 230pm.
Corrective Action(s): Cut mushrooms are a potentially hazardous food -maximum holding time at room temperature is 2 hours. Discarded the mushrooms.
You will need to time track any potentially hazardous foods at room temperature --> such as a whiteboard, stickies indicating when to throw out leftovers

This is the third consecutive routine inspection where the dishwasher is not working properly:

Correction Order issued as attached -note the next enforceable step is ticketing and/or closure

You are to Manually Wash dishware in the meantime at your 3-compartment sink until the new chlorine vats are in.

Observation (CORRECTED DURING INSPECTION): Low temperature chlorine sanitizing dishwasher measured at ~0ppm sanitizer residual

Operator indicates chemical vat orders were late. The present sanitizer vat was low yesterday and it was topped up with water??
Corrective Action(s): If your dishwasher is inadequately supplied you must be Manually Washing at your 3-compartment sink as per the sign posted -this is only a temporary means while the washer is repaired/reequipped

Correction Order issued as this is the third consecutive routine inspection where the dishwasher is down.

NOTE: dishwasher vat was replaced while during inspection -re-tested at >50ppm chlorine
