Observation (CORRECTED DURING INSPECTION): -Employee washroom in use again -toilet operational and door not locked -you were specifically told to either remove the toilet and handsink and covert to another use OR lock room permanently

Handsink again not working -no soap and paper towels available

-Back storage and food preparation area again has no paper towels dispensed or pumped soap available -also stated in last routine inspection
Corrective Action(s): You are to disconnect the toilet and keep the room locked or convert to another use related to the restaurant -door was locked as a temporary fix

-Install a fixed paper towel dispenser and pumped soap dispenser at back sinks area next to back exit -operator placed pumped soap bottle and paper towel roll temporarily

-Some portions of the ceilings in the kitchen have holes where apparently there were leaks -operator indicates the pipes have been sealed
Corrective Action(s): Remove the duck tape dishwash the ladel -all utensils must be easily cleanable

-repair kitchen ceilings -will assist the operator in discussion with the landlord here-
