Observation: STILL seeing a few mouse droppings throughout the premise:
-around the boiler heaters
-electrical room -a dead mouse in one of the snap traps here
-storage room
Corrective Action(s): You must be doing a more thorough cleaning of the premise on a daily basis
-Check EACH MORNING with a high powered flashlight (phone light is too weak) the entire premise for rodent activity -this includes ALL parts of the premise not just the food related areas

Observation: Floor drain in the production area is missing the grate -this is a possible pest entry point
Corrective Action(s): Replace and secure the grate on the floor drain (showed operator the other floor drain with the grate)

Corrective Action(s): The floor areas in production area need to be smooth and easily cleanable -you will need to repair these areas pointed out to the operator
