Observation:
Observation #1: Ice machine is beginning to look grungy on the outside and inside. Mould growth beginning in corners. [Date to be corrected by: end-of-day]
Observation #2: The insides of front drawers are beginning to show debris accumulation. [Date to be corrected by: January 31, 2019]
Corrective Action(s):
Corrective Action #1: Turn off the ice machine. Discard all ice. Clean the ice machine as follows: 1. Soap and warm water 2. Rinse 3. Quats sanitizer or 100 ppm bleach solution 4. Air dry. Do this every 1-2 months to maintain the ice machine.\
Corrective Action #2: Clean all of the drawers and shelving units.

Observation: Fruit flies observed in staff bathroom/chemical storage room. Used cloths are stored in this room for up to a week before being sent away for cleaning. [Date to be correcte dby: January 31, 2019]
Corrective Action(s): Fruit flies are drawn towards moist surfaces in order to reproduce. In order to reduce the number of fruit flies, the moisture sources must be removed. Suggestions: Bleach the mop heads weekly (destroy any food source, eggs inside). Seal up the used cloth bag with a more secure lid. Conduct a deep clean of this room.

Observation #1: Dust is accumulating on the ceiling fixtures, including the venting and light fixtures. [Date to be corrected by: January 31, 2019]
Observation #2: Spilled foods observed in two areas: spilled sugar on the floor underneath a shelf in the kitchen, spilled coffee beans behind one of the brewers in the front. These foods will attract pests. [Date to be corrected by: January 31, 2019]

Corrective Action #2: Wipe down the ceiling fixtures of all dust. Increase the frequency of dusting to prevent build-up.

All handsinks equipped with hot and cold running water, liquid soap and paper towel *Note: Staff bathroom handsink had a missing soap dispenser (corrected during the inspection).
Quats sanitizer solution at 200 ppm; available in spray bottles; test strips on site
Low-temperature chemical dishwasher reached 120F and achieved 50 ppm chlorine residual at the final rinse cycle. Test strips on site.
All coolers operating at </= 4C; Freezers operating at </= -18C; Hot-holding at >/= 60C
Temperature records maintained; dishwasher sanitizer records maintained
Foods are stored 6" off the ground, covered to protect from contamination

General sanitation is adequate
No signs of pests observed. A pest control contract is in place, 1x/week. Many pest control devices throughout the premise. Staff are aware of building's pest issues and importance of covering all foods, cleaning up all spilled foods.
