Observation: Rear area of premise, walk-in cooler exterior ceiling and walls deterriorated. lssue was to be repaired and has been outstanding for a year.
Walkin in cooler cooling unit and shelves soiled. Mould observed on shelving.
Floor tiles are cracked and grout areas have accumulations of debris.

Corrective Action(s): Rear are of premise to be repaired.
Clean and disinfect walk-in cooler.
