Observation: Rodent infestation observed. Evidence of rodents - droppings - observed throughout facility.
Currently facility inspected once per week.

Observation: Facility has hole along front door leading into facility.
Currently proofer is not in use. Large amounts of rodent droppings on bottom of proofer. This may be a site of activity.
Old equipment cluttering up facility. Old equipment makes cleaning difficult.
Food debris observed on floor and in crevices of equipment.
Box of nuts opened observed on lower shelf next to dishwasher.

Corrective Action(s): Fill all holes with non-chewable materials.
Seal all gaps in doors. Replace door sweeps
Any old equipment not in use should be removed and placed in storage.
Any old equipment to remain at site needs to be cleaned and regularly cleaned as rodent droppings are observed on old equipment.
Ensure opened boxes of food items are transferred to pest proof containers.

Observation: Rodent droppings observed throughout facility:
- Floor (under dishwasher, in front of proofer, back door, etc.)
- Lower shelves
- On old equipment
- On rolling rack lower portion
- Under stand mixer at back area next to garage door.
- Large quantity of rodent droppings observed on floor throughout facility.

Cleaning required due to food debris and grime in the following places:
- Oven
- Floor in front of proofer
- Lower shelves
- Dry goods fronts of containers
- Rolling racks
- Old equipment

- Dishwasher achieving final sanitizing rinse of greater than or equal to 71 (77.4) Degrees Celsius ppm at the dish
- Sanitizer solution in use throughout food preparation areas and at concentrations of greater than or equal to 200ppm QUAT
- QUAT Test strips available for use
- One functional sink plugs available for use observed
- Hair restraints in use
- Back door closed during inspection and tight fitting
- Light covers in place
- Cooler handles and seals in good sanitary condition
- Staff Personal items kept separate from kitchen equipment
- FOODSAFE level on or equivalent Certified staff present during inspection

NOTE: Facility only has one sink plug. Obtain additional sink plugs in case of loss.
