Observation: Water from floor splashed all direction in the back kitchen area. Food contact surfaces were visibly soiled.

Observation: Observed 5-6 flies throughout the facility. The back door was wide open for floor pressure washing.
Corrective Action(s): Implement integrated pest management (IPM) approach. Improve sanitation. Remove pest attractant. Actively work with pest control company to discourage pest issue. Correct this by: immediately.

Observation: Observed large amount of standing water throughout the facility from pressure washing. Back door is widely open and not screened.
Corrective Action(s): Remove all standing water which can attract pest. Do not open the back door for prolonged period. Correct this by: immediately.

Observation (CORRECTED DURING INSPECTION): Multiple cardboard boxes of take-out container/bags were soaked from pressure washing. Boxes with take-out containers and bags that cannot be washed and sanitized were discarded.

Closure order was issued.

Observed pressure washing of back kitchen floor while active food handling was in place in both front and back of the premise. Water from pressure washing splashed up to approximately 1m high. Observed water splashed on to food contact surfaces (e.g., egg station and donut station) and dry storage shelves. Multiple food and non-food items were directly and indirectly exposed.
