Observation (CORRECTED DURING INSPECTION):
Only two hand washing stations available in the facility and both were not fully equipped.
1. Hand washing station by cookline lacked paper towels.
2. Hand washing station in front lacked paper towels.
Corrective Action(s):
Place paper towels in dispensers at both hand washing stations. Ensure hand washing stations are fully equipped with liquid soap, paper towels and hot/cold running water at all times.

Observation:
Floor drain in front of hot holding cabinet for chicken strips is unsecured.
Corrective Action(s):
Secure floor drain with screws to prevent rodent entry through pipes. To be corrected by September 13, 2017.

Observation:
1. Build up of grease found underneath cookline/grill.
2. Debris has started to accumulate under equipment.
3. Rotting tomato slice found on dry storage shelf. Several fruit flies were present on the rotting tomato slice.
4. Light fixture above dry storage area has several dead flies/bees inside.
Corrective Action(s):
1. Clean underneath cookline/grill.
2. Clean underneath all equipment and ensure regular cleaning/sweeping/mopping is done all the way to the back wall.
3. Remove rotting tomato slice in dry storage area. Ensure shelves of dry storage area are frequently cleaned.
4. Clean out dead flies/bees from light fixture above dry storage area.

All to be corrected by September 13, 2017.

- QUAT sanitizer = 200 ppm.
- High temperature dishwasher sanitizing with >71 degrees Celsius at dish level.
- No evidence of rodent activity at the time of inspection.
