Report of Findings
On November 21, 2022, Licensing received a complaint alleging concerns with the physical facility.

The complainant provided the following details:
- The facility has black mold on the ceilings of the third floor bathrooms
- An aluminum gutter is hanging off the side of the roof
- The ceiling is leaking on the third floor near the skylight window

All areas of the complaint were thoroughly investigated. Evidence was received and reviewed from interviews, correspondence with the facility, and inspection.

The following contraventions were determined:
Residential Care Regulation - Section RCR 22(1)(b) - Maintenance

Observation: A section of the gutter was observed to be detached from the side of the roof and left hanging off the side of the building, and the facility had a ceiling leak, as confirmed by a PIC who resided on the third floor.
Corrective action: Corrective action required.
