Observation (CORRECTED DURING INSPECTION): Three spray bottles labeled as sanitizer contained no detectable chlorine.
Corrective Action(s): Bleach water sanitizer was remade and measured at 100 to 200 ppm chlorine. Ensure sanitizer is always available at the correct concentration for effective sanitizing of food contact surfaces (e.g. 100 to 200 ppm chlorine). It is recommended to obtain test strips to verify chlorine concentration.

Observation (CORRECTED DURING INSPECTION): The following was observed:
1. Plastic scoop was stored within mayonnaise container. Scoop handle in contact with mayonnaise.
2. Container of dried mushrooms was uncovered. Mushrooms stored on lower shelf near cook line.
Corrective Action(s): Ensure scoops are stored in a manner that protects food from potential contamination (i.e. scoop handles not in contact with food product). Handles are frequent contact surfaces and may potentially contaminate food.
Ensure all foods are covered to protect from potential contamination.

Observation (CORRECTED DURING INSPECTION): Receiving/back door observed to be propped open approximately one to two inches at time of inspection.
Corrective Action(s): Receiving/back door closed at time of inspection. Ensure doors remain closed to prevent potential entry of pests such as insects and rodents.

- Bleach water sanitizer available in labeled spray bottles and in buckets at 100 to 200 ppm chlorine*
- High temperature dishwasher measured 73 degrees C at the utensil surface during the final rinse (minimum required: 71 degrees C at the utensil surface)
