Observation: One hand sink not in operation due a leak. Operator stated this would be repaired early next week. Additional hand sink is within 4 ft and is in working order.
Several fluorescent light lights in ceiling fixture not working. Cover missing on both fluorescent fixtures. This is an OUTSTANDING ITEM listed in the report on July 10, 2019 to be corrected by November 10, 2019.

*Monitoring logs not being maintained. Ensure logs are recorded daily. Logs to include dishwasher data ie wash temperature and chlorine concentration.
Dishwasher at 120F and approx. 65ppm chlorine. Soap dispensed into wash cycle.
One front cooler not in use and is being used instead for storage
Meat slicer is clean
