Followup inspection to Rpt # VCHN - AFGTFK:

Pest control company provided services to address cockroach infestation November 7, 2016. Further services to be provided within 2 weeks. Ensure glue traps are placed in kitchen, dry storage room, mop sink/hot water tank room, and at back exit area.
Premise has been thoroughly cleaned and sanitized with focus underneath/behind equipment and all hard to reach areas. To reopen on November 9, 2016. Prior to reopening sanitize all food contact surfaces and equipment with 100ppm bleach solution.
Advised by operator that 2 cooks are taking FOODSAFE level 1.
Hand sink provided with liquid soap in pump style dispenser. Operator to purchase and install a wall mounted dispenser.
Interior of ice machine cleaned and sanitized.

Note: clean premise on regular basis. Follow your sanitation plan.

Closure ORDER issued on November 7, 2016 has been rescinded. Premise granted health approval to reopen.
