Observation: - Open containers of spices/ sauces/ oils on cart beside cooking equipment may have potentially been exposed to cockroaches.
- Open food in service area steam table may have potentially been exposed to cockroaches.
Corrective Action(s): - Discard above noted food items. Open food items are not to be saved for later use.

Observation: - Cockroaches (in all life cycle stages) observed throughout the premises:
- Under and between wall and equipment/ shelving in dry storage room .
- Along floor/ wall edges behind kitchen prep counter, prep sink, and prep cooler.
- Along floor/ wall edges behind cooking equipment.
- On floors and walls in kitchen and storage room.
- On floors in front service area.
- On walls between cooking equipment and walk-in cooler.
- Under food containers and items stored on kitchen prep counter.
- On food prep surfaces/ food contact surfaces - kitchen food prep counter, prep cooler cutting board, dishes stored on prep cooler.
- Evidence of cockroaches observed (brownish irregularly shaped smear marks) on wall edges/ joints.
Corrective Action(s): - Remove and clean up cockroaches and eggs.
- Eliminate food sources (grease, food debris, paper products, cardboard, cloths) and water/ moisture sources.
- Store all items at least 6" (15 cm) off the ground to facilitate cleaning and pest monitoring.
- Sort miscellaneous storage items (e.g. milk crates jammed with miscellaneous items and food equipment, storage cart, etc).
- Remove all items not required for the food premises operation.
- Ensure all areas of the premises are accessible for pest control service. Pull out equipment and shelves so that wall/ floor edges can be accessed.

Note: Operator contacted a pest control company after the last routine inspection. Premises was serviced this morning by a pest control service technician.

- Grease and food debris accumulation between, behind, under cooking equipment.
- Top of walk-in cooler.
Corrective Action(s): - Thoroughly clean above noted areas, all areas listed on Violation 304, and all other surfaces and hard to reach areas (cracks, crevices, edges, corners, etc).
