Observation: Mice droppings still noted in back area on wooden shelves with a wood kick plate, and sunlight can clearly be seen around edges of back door.
Corrective Action(s): Middle wooden shelf has a piece of wood along the front, remove this piece to assist in cleaning and to monitor for pest activity under this shelf.
Seal all holes in the premises including around/under back door, no sunlight should be seen around the door frame.
Clean and sanitize all mice droppings and continue to monitor.

Comments

- Food was found to be put away with covers
- Cleaning has been done in most areas, continue to clean above pass through window and under shelving in back area.
NOTE: Tile wall behind grill above the pass through window as paint and part of drywall are coming loose making it difficult to clean the grease off.
- Personal items are store separately from food areas
- Missing and broken tiles have been repaired in most places
- Hand sink had liquid soap, and paper towel
- Equipment appears to be maintained in clean and sanitized manner.
NOTE: Submit a copy of the new floor plan for review, include how the will food items will be transported from the other location as the Food Safety Plan has identified these foods as potentially hazardous.
