Observation: 1. Soap dispenser in the men's washroom was broken.
2. Paper dispensers at all hand washing stations were not being used. Rolls of paper were left beside hand washing stations.
Corrective Action(s): 1. Fix / replace the soap dispenser in the men's washroom. In the meantime, obtain a bottle of liquid hand soap for the men's washroom.
2. Intall paper rolls into the paper dispenser. Ensure paper dispensers are used. Correct by: Immediately.
