Routine site visit.
- Services provided: Fitness with massage chairs and tanning - one standing unit and one bed.

Client Safety:
- Staff operation conducted at the front reception area.
- Staff inform clients regarding the safe use of tanning devices. Skin type analysis forms available on site.
- Tanning machines are activated at main desk and are timed according to the skin photo typing schedule.
- Emergency shut-off button present in tanning area.
- Client brings UV safety goggles, which are evaluated by the operator prior to use. Tanning services not allowed without protective eye wear..
- Protective barriers are present and in good repair.

Warning Signs
- UV Radiation warning signs and Age Requirement signs (from Ministry of Health) clearly posted in each of the tanning rooms.
- Age verification conducted during membership purchase.

Disinfection
- Quaternary ammonium compound-based disinfectant used for low-level disinfection of tanning beds. Disinfection conducted by staff and patrons.
- Spray bottles located in each tanning room are labelled as "disinfectant" with > 200 ppm quats.

Sanitary Facilities
- No concerns noted.
- Routine cleaning of shower stall/shower curtain cleaning discussed.
