Corrective Action(s): Rolling cart removed. Ensure that hand sinks are not blocked so they are accessible for employees to wash their hands.

Observation: Decayed rodent, dead insects observed on glue board in back room area behind refrigeration pipes.
Corrective Action(s): Contact pest control company to remove the glue board. Disinfect the area with 200 ppm chlorine bleach and clean the area. Ensure that area is monitored.

Observation: 1. Long-standing food debris observed in walk-in freezer in back room.
2. Long-standing debris observed between the pallet guard and wall in the receiving area.
3. Food debris observed on the floor around the compost area.
4. Long-standing debris observed under the stand-up fridges in the PC Express area.
Corrective Action(s): To maintain the facility in sanitary condition and prevent the attraction of pests, clean all affected areas of long-standing debris.
