Observation: Dry food residue still present on the dough flattener and the stainless steel worktable.
Corrective Action(s): Clean and sanitize all food contact surfaces in the kitchen. Correct this by: immediately.

Observation (CORRECTED DURING INSPECTION): The hand washing station by the entrance of the kitchen did not have single use paper towel. A roll of paper towel replaced at this time.

Observation: Observed damaged tile grouts by the kitchen floor. Observed grease/soiled water seeping out from damaged grouts when pressure was applied.
Corrective Action(s): Damaged grouts are to be replaced.

Observation: Sanitation improvement required at the following areas:
- Layer of grease on kitchen floor.
- Oil spill in the back of stove.
- Thick sauce/oil layer on the white shelves (located along the wall of kitchen).
- Standing water at the entrance to dishpit and on grease trap cover.
Corrective Action(s): Clean and sanitize above mentioned areas.

Follow up inspection conducted:
General sanitation improved; however, more areas require cleaning attention (see violation for details)
Reviewed pest control report and kitchen hood cleaning report.
Cockroach traps replaced after the cleaning by the pest control company.
Low temperature dishwasher's sanitizing cycle measured at 100 ppm chlorine at the plate level.

Closure order not rescinded at this time. Follow up inspection to be completed once cleaning is completed.
