Observation (CORRECTED DURING INSPECTION): * Sanitizer concentration at dispenser was not adequate (<200 ppm or negligible), and one sanitizer spray bottle being used to sanitize food contact surfaces did not have active sanitizer in it; operator changed the main sanitizer jug/dispenser bottle and then the sanitizer concentration was 200 ppm at dispenser and could be used to fill buckets/spray bottles
Corrective Action(s): * Ensure all sanitizer used for food contact surfaces and sanitizing dishware meets the minimum of 200 ppm for quaternary ammonium sanitizer (concentration); CHECK daily both buckets and spray bottles to ensure proper concentration being met

Observation: * Some food equipment on shelving above dish washing area was found being stored below the wiring of video cameras, etc., and this is unsanitary; operator reminded to keep food equipment stored properly and protected from contamination; Also, staff bag and coat found stored in food area along with paper work and these areas are designated food prep areas not staff storage or paper storage areas; operator addressed these issues
Corrective Action(s): * Ensure all food equipment protected from conatmination in storage areas; also, ensure staff personal items kept in designated area away from food preparation/handling areas(paper stuff too); Also, reorganize dry storage shelving in back so that personal stuff in designated area-- food products to be kept separately from staff items and other non-food stuff

Observation: * Flies were found in back prep areas and fly controls were not in place; discussed providing food grade pest control devices (eg. blue light zapper)
Corrective Action(s): * Provide food grade fly control devices(eg. fly tape and swatters not acceptable) to control fly issues

* Food was covered in storage and labeled
* Sanitizer was not active at dispenser unit so operator changed main sanitizer jug/bottle and then it had the 200 ppm concentration(quaternary ammonium)
* Hand washing station had liquid soap and paper towels
* Staff were dressed appropriately and had hair protection
* Follow up inspection to be done for outstanding issues
