Observation (CORRECTED DURING INSPECTION): Container of cooked rice stored at temperatures greater than 4 degrees in Microwave unit. Staff explained the rice had been removed from cooler unit and placed in microwave until rice runs out in front. The rice would then be reheated and dumped into insert. Rice had not yet been reheated. CORRECTED DURING INSPECTION - Rice moved back into cooler unit until such time as it would be needed.

Corrective Action(s): DO NOT store rice in microwave. DO NOT store rice, potentially hazardous food item, at temperatures greater than 4 and less than 60 degrees Celsius.
Move cooked rice to microwave only when front insert runs out.

Observation: Raw squid on shelf containing cooked potatoes and salad. Squid thawing and dripping. Remaining raw squid on lower shelf of same shelving unit.

Corrective Action(s): Relocate raw squid to proper shelf. All raw meat and seafood is to be stored below ready to eat food items.

Observation: Rodent droppings found on lower shelf of microwave unit behind black garbage bag boxes.
Rodent droppings found on floor of dry storage unit.

Corrective Action(s): Inform Pest technician.
Add additional traps to both restaurant and storage room.
Cardboard boxes of potatoes and onions in storage room need to be moved into pest proof containers. This was discussed with all tenants of Storage Room 4. All food items not in pest proof containers need to be stored in pest proof containers. Current cardboard has large holes which can allow mice to enter into boxes.
