Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care & Assisted Living Act (CCALA), the Residential Care Regulation (RCR) and the relevant Director of Licensing Standards of Practice (DOLSP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records and information provided by the facility staff at the time of inspection.

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31290 - RCR s.22(1)(b) - A licensee must ensure that all rooms and common areas are (b) maintained in a good state of repair.
Observation: It was noted that
Dining area-
The cement poles in the dining area are scuffed and missing paint
Baseboards and chair rails are scuffed and missing paint
Dining chairs are wood and worn down to the wood ( not able to be cleaned sufficiently)

South
Nursing station has a broken cupboard door and counter is missing laminate and wood is showing

North
Storage closet with plastic bifold doors is broken and does not close properly

BSTN
damage to the walls and paint around the nursing station
6 doors missing kick plates
shared area washroom has a hole in the drywall

Corrective Action(s): ensure all rooms and common areas are maintained in a good state of repair

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31300 - RCR s.22(1)(c) - A licensee must ensure that all rooms and common areas are (c) maintained in a safe and clean condition.
Observation: The vaulted ceilings in all areas of the building have visible dust on them
Corrective Action(s): ensure that all areas are maintained in a clean condition
Date to be Corrected: July 12, 2021

CARE AND/OR SUPERVISION: 34470 - RCR s.73(2)(c) - In addition to the requirements under subsection (1), the following conditions apply to the use of a restraint under section 74(1)(b) [when restraints may be used]: (c) the use of the restraint, its type and the duration for which it is used must be documented in the care plan of the person in care.
Observation: PIC in Room CL20 has a child gate up that the PIC is unable to open or close independently and no safety plan has been approved for the use of this restraint
Corrective Action(s): Ensure that all restraints have approved safety plans documented in the care plan
Date to be Corrected: Further discussion determined that the gate is to keep other residents out and that the PIC is unable to ambulate independently and would not be able to use the gate independently. Care plan was updated to include the details of the use of the gate for the residents personal protection - corrected at the time of inspection

CARE AND/OR SUPERVISION: 34780 - RCR s.82 - A licensee must ensure that the care and supervision of a person in care is consistent with the terms and conditions of the person in care's care plan.
Observation: 1 PIC has a turning schedule documented in their care plan that states the PIC is to be repositioned every 2 hours while in bed, there is no documentation to show that the staff are following the repositioning schedule as described in the care plan

Corrective Action(s): ensure that care and supervision is consistent with the care plan and documentation is available for review
Date to be Corrected: corrected during inspection and an audit of other care plans has been started to review repositioning
