Observation: Debris deposits on floor to either side of new stove / grill units.
Mousing droppings observed on the floor within the kitchen at the following locations: around dough mixer, under wheeled bulk dry food bins, inside breaker box of hot holding unit, under cart below fire extinguisher, behind kettles, under clean dish storage shelving, under dishwasher prerinse sinks, under 3 compartment sinks, under banana storage shelves, in corner of janitorial station and in corner at back entry double door.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning.
Premise cleaning to be conducted on a daily basis to maintain a high level of sanitation and discourage pest activity.

5 cracked, chipped plastic bus pans
Corrective Action(s): Repair 2 compartment sink sanitizer feeder to provide 200 ppm Quaternary Ammonia.
Discard all cracked, chipped plastic bus pans.
