Observation: High temperature dishwasher - average sanitizing temperature 66 C after 10 cycles.
Corrective Action(s): High temperature dishwasher is to be repaired to provide a sanitizing cycle that meets or exceeds 71 C at dish level.

Until repaired, all dishware and food untensils to be washed in dishwasher as normal. After dishwasher cycle, all dishware to be transferred to sanitizing compartment sinks filled with 200 ppm Quaternary Ammonia and immersed for 2 minutes then allowed to air dry.

Use single use plates for meals to minimize volume of dishware requiring manual sanitizing.

Observation: Debris deposits on floor to either side of new stove / grill units.
Mousing droppings observed on the floor within the kitchen at the following locations: around dough mixer, under wheeled bulk dry food bins, inside breaker box of hot holding unit, under cart below fire extinguisher, behind kettles, under clean dish storage shelving, under dishwasher prerinse sinks, under 3 compartment sinks, under banana storage shelves, in corner of janitorial station and in corner at back entry double door.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning.

Premise cleaning to be conducted on a daily basis to maintain a high level of sanitation and discourage pest activity.
