Observation (CORRECTED DURING INSPECTION): Blocked handsinks in butcher room (2).
Equipment and supplies blocking access.
Corrective Action(s): Ensure all handsinks are free of obstructions at all time during operation. Butcher room was not in use during inspection however both sinks were completely blocked. Review with staff the importance and use of handsinks in a food facility and keeping these sinks free of obstructions.

Observation: Mouse droppings noted on various upper shelves in warehouse and under pallets.
Corrective Action(s): Ensure all areas of facility are routinely cleaned and monitored for pest activity. Pest control provided by Orkin and regular service provided. High activity areas should be reported to PC.

Observation: Cardboard liners noted on upper shelves above sinks in butcher room - liners appear stained, wet and soiled
Corrective Action(s): Ensure all shelves and surfaces in premises are smooth, durable, easy to clean and non-absorbent. Cardboard does not meet the requirements noted above. Remove all cardboard liners. If shelves require additional cleaning without absorbing materials then increase frequency of cleaning. Shelves are stainless steel which is an approved material - do not cover with liner.

Observation: 1.Large crack in the floor of butcher room, epoxy layer sealing concrete is lifting off. Water slowly moving between epoxy layer and concrete lifting more and more. Unable to properly clean floor, crack widening
2. 2 light fixtures burned out in Deli walk in cooler - very dark in room
Corrective Action(s): 1. Ensure there is a long term plan in place to repair floor. Floor must be smooth, durable, easy to clean and non-absorbent. Currently the large gap does not meet this requirement.
By next routine inspection a plan must be in place to repair/fix flooring in this room. Email EHO when plan is in place.
Correction date: Before April 2024

2. Ensure all areas for food processing and storage is maintained with suitable lighting, enough to monitor for pests and maintain sanitation. Replace/repair fixtures. Operator says they are slowly replacing florescent light fixtures with LED lighting.
