Observation: 1) Encrustation noted on the dish racks on top of the two compartment sink.
2) Dried cucumber pieces noted on a vegetable slicer.
Corrective Action(s): Clean and sanitize areas noted in the infraction.

Observation (CORRECTED DURING INSPECTION): A staff took off her single use gloves to handle another non-food related task. She used same set of gloves without washing hands.
Corrective Action(s): Always wash hands in between tasks even if gloves are worn. A staff washed her hands then put a new pair of gloves.

209 - Food not protected from contamination [s. 12(a)]
Observation: 1) An open bag of white uncooked rice stored in its original package.
2) Open containers of ginger and vinegar stored directly on floor.
Corrective Action(s): 1) Provide a container with tight fitting lid for all opened bags of foods to protect food from contamination.

Observation: Fruit flies noted in the sushi bar area and in the back kitchen. Fruit flies traps noted on the premise and it appears to be effective at catching fruit flies. Orkin's latest report (July 30) did not mentioned about fruit flies.
Corrective Action(s): Increase general sanitation to discourage pest activity. Look for any standing water or decaying food debris. Install more fruit fly traps.
