Observed excessive accumulation of ice on a right side in the cavity of the cooler.
Corrective Action(s): Please repair/service the under counter cooler. Temperature of the cooler/in the cavity/ must be = < 4.0 C.

Food items are stored safely at the time of inspection (in plastic insert containers with lids, dated, and labeled & off the floor);
Hot holding units > or = 60 C.
Three compartment sink - satisfactory;
Quat sanitizer available at 200 ppm concentration in a wall mounted dispenser.
Test strips - available;
Surface sanitizers available in designated buckets with presoaked wiping cloths at 200 ppm Quat solution in the back of the kitchen and front.
Note: Surface sanitizer in spray bottle(s) must be tested by Quat test strips and replaced as needed (must be 200 ppm).
Dry storage area - organized;
Washrooms - clean; supplied with liquid soap and paper towel;

Note: Small amount of food in the under counter cooler (a soup with ice in it; two food items at 1.1 C and 3.2 C).
