HAZARD RATING FOR YOUR FACILITY: Low (≤15) Total Score = 6
Critical Hazards: There are no critical hazards.
Non-Critical Hazards: Total Number: 2
304 - Premises not free of pests [s. 26(a)]
Observation: Inadequate pest control measures in place.
Corrective Action(s): Operator states the premise is currently serviced by Care Pest on a monthly basis, however the 1-year contract is coming to an end. Retain the services of a licensed pest control company with a contract that has a minimum duration of 6 months with servicing at a minimum of once per month, to help control rodents in the premise. Mechanical and/or sticky traps shall be used throughout the premise to control and monitor for mouse activity. Provide a copy of the new pest control contract to Fraser Health for review. Operator shall retain a copy of every pest control service report onsite for record keeping and reference.

Date to be corrected: January 31, 2019
Violation Score: 3

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: Materials used in storage areas and old items may allow for harbourage of pests.
Corrective Action(s): The black-colored, perforated foam liners used on the shelves for glassware shall be replaced with light-colored liners. Black-colored liners can make it very challenging to monitor for rodent droppings. If droppings are found on/near liners, the liners shall be discarded.
Do not use cardboard to line shelving as they cannot be cleaned and can attract pests.
Any items stored in the premise that are not needed for day to day operations shall be removed from the premise. Cluttered storage areas can allow for harbourage of pests.

Date to be corrected: December 17, 2018
Violation Score: 3
