Observation (CORRECTED DURING INSPECTION): Green cutting board next to 2 comp sink in back prep room appears dirty and stained.
Corrective Action(s): Ensure all food contact surfaces are cleaned and sanitized after every use.

Observation: Office and back storage area are very cluttered with boxes/bags of food products
Corrective Action(s): These areas require organization and for foods to be stored on adequate shelving to ensure rodent harborage areas are not created.

Observation: Food debris and dirt is noted on the floor especially in the hard to reach areas such as along the sides of walls, behind equipment, under bulk food bins and underneath milk crates (which are being used to store food items off the floor)
Mold and dust debris accumulation on the ceiling in walk in cooler and around fans.
Corrective Action(s): Thoroughly clean the floor in the entire premises to remove all dirt, debris and mold

Observation: 1.Cardboard is being re-used multiple times and is showing wear and tear, old food debris and filth.
ex.Large water melon box holding whole water melons has notable melon debris and splatter throughout box which is being transferred onto food
2. Cans of tomato juice being used to hold up a lower shelf in prepackaged dry food section.
Corrective Action(s): 1.Ensure cardboard is used once and then discarded/recycled.
2. Cans are not a suitable replacement for shelving support. Replace with proper equipment and materials.

No signs of pest activity were noted at the time of the inspection. Operator has contract with pest control company. Traps are present for monitoring.
Hot and cold running water was available
Bleach was present in jug for sanitizing produce prep area/sink.
