Observation (CORRECTED DURING INSPECTION):
- Observed multiple potentially hazardous food items stored in room temperature. These included: cooked garlic, cooked mushroom, marinated raw chicken, raw shelled eggs.
Corrective Action(s):
- Do not store them on counters. Ensure above items are kept in cooler at 4C or below.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION):
- Bleach sanitizer stripped out the test strip - this indicates that they are too strong.
Corrective Action(s):
- Ensure correct ratio is mixed to reach 100-200ppm bleach sanitizer (1 tbsp to 1 gallon of water). Equip a chlorine test strip. Bleach sanitizer was diluted to 200ppm solution at the time of inspection.

Observation (CORRECTED DURING INSPECTION):
- Observed a bucket of chopped cabbage underneath soap dispenser by the handsink inside kitchen.
Corrective Action(s):
- Do not place food items at locations prone to contamination.

Observation (CORRECTED DURING INSPECTION):
- Observed frozen prawns and large intestine being thawed in room temperature.
Corrective Action(s):
- Ensure to thaw frozen food items using one of the following methods: inside the cooler, microwave, under cold water.

304 - Premises not free of pests [s. 26(a)]
Observation:
- Live cockroach found by the drink cooler at the bar.
- Dead/live cockroaches found on insect monitor traps inside the kitchen.
- Dead cockroach found inside the bar cabinet.
- Mouse droppings observed on the dishwasher inside the kitchen, on the floor next to the stand up cooler in the back storage area, on the stainless shelving at the bar.
Corrective Action(s):
- Biweekly pest monitoring done by professional pest control company. Recent report dated August 21, 2023 indicates cockroach and mice activity. Signs of heavy pest infestation noted at the time of inspection.
- Treatment required for cockroach control. Inspection required for building integrerity to identify entry/harborage points of cockroach and mice.
- Provide service report to the inspector for review.

Observation:
- Dust build up observed on fan covers inside coolers (bar & kitchen).
- Dust build up observed on ceilings and around the ceiling vent inside the kitchen.
- Mould observed on the wall around the sinks at the front.

- Clean areas noted above.

Observation:
- Observed raw meats stored in non-food grade containers. Containers have rough surface from heavy usage. Containers are labeled as "Hybrid plastic - shoe box".
Corrective Action(s):
- Discard these containers. Do not use non-food grade and single use disposable containers to store food items.
