Health Inspector to visually inspect Pest Technician invoice to see what exactly was done at facility. A professional company MUST be called as this infestation can NOT be managed by operators.

- Dishwasher achieving final sanitizing rinse of greater than or equal to 50 ppm chlorine at the dish
- Two functional sink plugs available for use observed
- Hair restraints in use
- Light covers in place
- Cooler handles and seals in good sanitary condition
- All containers used to store lids/utensils/etc. in good sanitary condition
- No evidence of rodents during inspection
