Observation: Ice bin of ice machine is dirty and has visible mold/slime.
Corrective Action(s): Discard ice that is currently in bin and clean and sanitize bin of ice machine. Do not resume use of ice machine until intrerior of machine this is complete. Correction date: Aug. 24/17

Observation (CORRECTED DURING INSPECTION): Handwashing station in dishwashing area being used to rinse produce.
Corrective Action(s): Do not use handwashing stations for food prep. Ensure handwashing stations are always available for handwashing and not used for other purposes. Use food prep sink for rinsing produce.

Observation: Mouse droppings noted in the following locations:
-under prep tables in kitchen along left side wall
-in dry storage room under shelving
-under sink in front waitress station
Corrective Action(s): Remove all items from floor and store on shelving. Clean up all droppings and disinfect areas. Keep floor area clear to facilitate cleaning and monitoring. Continue to monitor for future activity. Supervisor reported Pest Control contract in place Catch traps and adhesive traps present. Supervisor could not locate Pest Control reports at time of the inspection. The reports for August must be provided at the follow up inspection. Correction date for cleaning: Aug. 30/17

-Coolers were 4C or colder
-Food stored in coolers were covered with lids and plastic wrap
-Walk-in freezer was at -19C
-Hot holding of miso soup, rice, beef was >60C
-pH of sushi rice was 4-4.5 (measured with test strips).
-In use utensils being stored in ice water (correction from previous routine inspection)
-Quats sanitizer (200ppm) present in spray bottles for sanitizing food contact surfaces. *Ensure all staff are knowledgeable about sanitizer use and what product is being used. Some staff members were unaware where product is located and what it is.
-50ppm chlorine detected in rinse cycle of dishwasher to sanitize dishes. *Obtain chlorine test strips to verify chlorine residual
