2. Vegetables on food self-service line are not equip with their own utensil.
3. Repeat: Staff personal items observed stored on shelf above dry goods.
Corrective Action(s): 1. Ensure bowls with no handles are used to scoop dry goods to prevent cross contamination.
2. Ensure there is one utensil available for each item on the self-serve line to prevent from cross contamination and prevent food allergies.
3. Store personal items in a location separate from food and food contact surfaces either below food items, or placed in a container to prevent cross contamination.
