Observation: 1. Equipment on the cooking line is soiled with food and debris including fronts of equipment, legs and wheels, cooler drawer handles, inside cooler drawers, shelves and warmers.
2. Knives and utensilson the line stored in ice water are not being washed throughout the day.
Corrective Action(s): . 1Clean all equipment thoroughly and maintain in a sanitary condition at all times.
2. Knives and utensils used on the line must be washed and sanitized thoroughly every 2-4 hours.

Observation: No paper towels available at bar sink. Soap dispenser in rear kitchen is not working.
Corrective Action(s): Mount a paper towel dispenser at bar and keep stocked at all times. Repair or replace soap dispenser in rear kitchen.

Observation: Walls, floors and ceilings are soiled with food and mold including the floor of the walk-in cooler, the ceiling above the cooking line, and the walls behind the ice machine and dishwasher.
Corrective Action(s): Clean these areas throughly and frequently to maintain good sanitation at al times.

Observation: Large cutting boards on the line adn small cutting boards in rear kitchen are heavily stained and scored.
Corrective Action(s): Repair or replace all damaged boards on or before June 21, 2016.
This is a repeat violation. Failure to correct this violation by June 21st will result in a CORRECTION ORDER being issued.
