304 - Premises not free of pests [s. 26(a)] Observation: Mouse droppings observed in the following locations: along wall at back door entrance, on top of carpet lined wheeled cart for boxes of sauce mixes, underneath ovens shelving unit, under metal wire shelving unit across from donut station, and under right front of house handsink. 4 adult cockroaches and 2 juvenile cockroaches observed underneath 3 compartment sink station. Corrective Action(s): Premise sanitation must be improved immediately and maintained on a daily basis to for hygiene and pest control purposes. All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces. Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning. Violation Score: 9 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)] Observation: Tape strips along baseboads behind front wheeled ice cart and next to front of house wall organizer have been chewed through by mice. Corrective Action(s): Chewed tape to be removed and replaced with pest proof strips. Violation Score: 3 311 - Premises not maintained as per approved plans [s. 6(1)(b)] Observation: Kitchen tile floor - grout is worn away between floor tiles, allowing for debris to collect and cockroaches to reside. Corrective Action(s): Kitchen tile floor - grout is to be replaced to allow for an easily cleanable surface. Violation Score: 3
