Observation: Kitchen entry door, beverage cooler and front service counter baseboards have spaces to allow the entrance of pests.
Corrective Action(s): Kitchen door to be equipped with weather strip door sweep to exclude pests.
Front counter baseboard to be repaired to seal counter to exclude pests.
Beverage cooler to be equipped with a pest proof barrier to exlude pests from entering beneath cooler.

Observation: Debris and mouse droppings observed in the following areas: behind chest freezer, behind metal storage units, behind proofer, corners of handsink counter, around garbage / recycling station, on floor around safe and under janitorial and dishwashing sinks.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine) after cleaning.

Premise cleaning must be conducted on a daily basis to maintain premise sanitation and discourage pest activity.
