Observation (CORRECTED DURING INSPECTION): REPEAT VIOLATIONS. Staff was using handsink to wash bean sprouts at the time of inspection.
Corrective Action(s): Staff removed bean sprouts. Educated the operator a staff that dedicated hand sinks are to be used for hand washing purposes only.

2) Utensils kept in warm water baths (rice scoop)
3) Raw proteins (for bone broth) found without any packaging in the chest freezer
4) Sliced vegetables observed to be uncovered in the walk in cooler
5) Food Observed on the floor
Corrective Action(s): 1) Discard ice in the ice machine and do not use ice until ice machine has clean and sanitized
2) Operator placed rice scoop in dishwasher at the time of inspection. Ensure to kept utensils in either dry conditions or in ice water to prevent potential microbial growth.
3,4) Ensure food is packaged in food grade material to protect it from contamination
4) Ensure to keep all food at least 6 inches off floor level to prevent potential pest attraction and facilitate in proper cleaning practices.
