Observation: Back hallway ventilation system has not been approved and appears dusty inside the ventilation shaft. Filters used are not designed for this makeshift ventilation system. This system takes air from the back door which is leading to the parking lot and is drawing it into the kitchen above the 3 compartment sink area.
Corrective Action(s): Call a Heating, Ventilation, Air Conditioning technician or equivalent and ensure that the premises is equipped with suitable air quality according to the regulations and code. This statement is to be provided to the health inspector.

-Follow up Inspection
-Pest control report provided stating that the seclira cockroach gel is "old one" and a verbal phone conversation with the pest control operator determined that it was not done by their company, but another company. This could not be confirmed with current documentation provided to inspector.
-Thorough cleaning of the premises was conducted during the inspection.
-Discard all tools used to clean off the brown gel like substance which is suspected to be seclira by an old company according to current pest control provider which has been under contract for at least 3 years according to the food premises operator.
-This incident has been referred to an appropriate agency for follow up.
-Continue to maintain premises in a sanitary condition as to not contaminate food or food contact surfaces.
-Other violations from previous report corrected.
-Correction order rescinded.
