Observation: 1. *REPEAT* (Since Nov 2023) - Frost build up observed in chest freezer.
2. Frozen lemons (3) observed to be stored in Richard D's chest freezer. Product not in container/packaging and was contaminated by frost build up.
**Corrected during inspection** Product voluntarily discarded by staff on site.
3. Ice machine observed to have stains on the interior.
Corrective Action(s): 1. Defrost, remove ice and sanitize equipment. If ice formation returns, contact refrigeration technician to have unit serviced.

Note:
Walk-in freezer and Chest freezers (Chapman, Nestle) is now used by grocery store adjacent to the bubble tea store.

Handwash stations equipped with hot/cold running water, liquid soap and paper towels.
3 compartment sink on site.
Bleach sanitizer available on site, staff mixing sanitizer during inspection, measured 100ppm

Storage:
Food products stored at least 6" off floor.
All food were either in original packaging or covered.
Storage area organized.
Chemicals and cleaners were stored separate from food preparation areas.

General:
Pest control company contracted for monthly inspections.
Valid decal displayed.
