Observation (CORRECTED DURING INSPECTION):
Undercounter cooler with inserts was at 13 deg.C. Cooked meat and cutted vegetables were at 11 - 13 deg.C of surface temperature. Staff Stated they placed the foods in the cooler 2 hours ago.
Corrective action: Transfer all food items from the malfunctioning undercounter cooler into the working upright cooler. Do not store any new potentially harzardous food to the malfunctioning cooler until it being fixed and maintained at or below 4 deg.C.

Observation:
Hot holding unit wasn't being turned on. Cooked meats stored inside were at 47 deg.C and 37 deg.C internal temperature. operator stated that the meats were placed in the hold holding units 2 hours ago when they open for operation.
Corrective action: reheat the meats and other food items in the hot holding unit to a minimum of 74 deg.C. Ensure Hot potentially harzardous foods are stored at or above 60 deg.C in the hot holding units.

Observation:
1) Grease and oil debris are noted on floorings, undercounter areas and cooler handles.
2) Grease and oil debris are accumulated on the vent hood. Next service date for vent hold was overdue 2 months ago.
3) oil grease and dirts accumulated on top of grease trap and below griller and deep fryer.
Corrective action:
1) and 3) Deep clean the premise to remove grease, oil debris, and dirts from the mentioned areas, ensure premise is being maintained in a sanitary condition by cleaning and Sanitizing the premise routinely on a daily basis.
2) clean and sanitize the vent hood to prevent fire hazard and food from contamination from dripping oil. Follow the cleaning schedule of the vent hood and ensure the vent hood is being deep cleaned by a professional cleaning company routinely.

Ketchup was being stored in its original opened can in the 2-door cooler.
Corrective action: Store ketchup in a food-grade container. Never Store foods in its opened cans to prevent leaching from the can contact with food.

Observation:
1) Handsink in the kitchen was broken that no cold running water are available for hand washing. liquid soap dispenser beside the hand sink is unable to be attached on the wall. operator stated that the hand sink will be repaired by the end of today
2) Undercounter cooler with insert was at 13 deg.C. No temperature log maintained and recorded for all refrigeration and hot holding units.
Corrective action:
1) Repair the handsink to allow employee washing their hands properly. Re-attach the liquid Soap dispenser on the wall

2) Repair the undercounter cooler with insert. Do not store in any potentially harzardous foods in the cooler until it's being fixed. Ensure the cooler is able to maintain at or below 4 deg.C.
