Observation (CORRECTED DURING INSPECTION): Various sauces including tom yum and laksa in the metal inserts all stored in a metal bin did not come in contact with ice cubes.
Corrective Action(s): Ensure that inserts of food come in full contact with ice water mix to keep the food at 4C or colder.

Observation: Some mildew inside ice machine.
Corrective Action(s): Ensure to clean the inside of the ice machine properly during scheduled cleaning. To verify next follow up.

Observation (CORRECTED DURING INSPECTION): Wet wiping towels left on the counter. Bleach sanitizer made after most of the food prep is done in the morning.
Corrective Action(s): Store wet wiping towels in 100ppm bleach sanitizer between uses. Always mix the bleach sanitizer in the beginning of the shift so all staffs are able to sanitize the food contact surfaces.

Observation (CORRECTED DURING INSPECTION): 1) A basket of cilantro was store under the sink next to the garbage can. 2) Bucket of rehydrated noodles, rice and cut vegetables were stored on the floor directly.
Corrective Action(s): 1) Store all food including garnishes in a food grade container with close fitting lid to prevent food from being contaminated. 2) Keep the food 6 inches off the ground.
