Observation: Internal temperature of food in the under the counter coolers with inserts measured at 8-10C. Potentially hazardous food, sliced tomoato, chopped lettuce, chopped onion was stored in the inserts from the morning.
Corrective Action(s): 1. Repair the coolers so they can maintain the internal temperature of the food at or below 4C
2. Potentially hazardous foods were discarded at the time of inspection.
Note: Operator stated that the coolers will be repaired as soon as the next day.

Observation (CORRECTED DURING INSPECTION): Sanitizer bucket is provided but no concentration of sanitizer was detected in the bucket.
Sanitation cloth was not stored in the bucket.
Corrective Action(s): Please ensure all sanitizer buckets are refilled every 4 hours or as needed and store wiping cloth in the bucket at all times.
