Observation (CORRECTED DURING INSPECTION): 1. Observed dirty scoops being used to scoop dry goods
2. Dirty knives stored in knive holder that is damaged
3. Outer skirts of dishwasher not in sanitary condition; observed accumulation of food debris
Corrective Action(s): Required operator to remove all dirty scoops and knives, clean and sanitize prior to re-use. Ensure to clean and sanitize them regularly. Dishwasher must be scraped, cleaned and sanitized.

Observation (CORRECTED DURING INSPECTION): Several staff in the kitchen at the time of inspection. Staff perfomring different tasks did not wash hands in between tasks

Observation: 1. Found items in the walk-in freezer where food was directly touching packages and boxes
2. Dirty scoops used to scoop dry goods
3. Fans used for air circulation was not in sanitary condition; extremely dusty and dirty. These fans spreading dust in the kitchen and contaminating foods
Corrective Action(s): Required operator to ensure that food is protected with appropriate covers and that each utensil is maintained in sanitary conditions. Fan usage is acceptable as long as they are maintained in sanitary conditions. Operator must clean the fans so that it does not spread dust and other debris into the air. Operator moved the items in the walk-in freezer and moved dirty scoops to the dishwashing area at the time of inspection

2. Under and on shelves where dry goods are stored
3. Underneath pizza oven
Operator indicated that monthly visits are made by pest management hired by the strata but no reports are provided. Last visit made 1.5 months ago
Corrective Action(s): Required operator to remove all droppings; ensure to wet them with bleach water prior to removing them. Clean and sanitize the area. Regular cleaning must be performed to maintain sanitary conditions as well as to monitor for pest activities
