Corrective Action(s): Defrost all PHF either in cooler over night or under cold running water ( in water proof packaging). Corrected at time of visit.

Corrective Action(s): Deep clean the entire facility.

-Dishwasher chlorine concentration >100 ppm
-Chlorine concentration in bucket for wiping rags is >100 ppm
-Hot holding >60 oC

** Install self closing device for washroom door.
** Ensure to monitor temp of all coolers and freezer (at least twice daily) chorine concentration for wiping rags and dishwasher at least once daily record in log sheet and keep on site for routine check by FHA as per request.

** Ensure to check Vaccination records properly (with pic id) for any dine in customers
