209 - Food not protected from contamination [s. 12(a)] Observation: 1. Stored food in walk-in cooler were not all covered and protected from contamination. Observed food itesm uncovered include bread, chopped green peppers and diced chicken. 2. Plastic bowls were used for scooping salt/sugar. The plastic bowls were observed to have accumulation of food debris 3. Plastic boards with debris and grease build-up were used to cover pizza trays Corrective Action(s): 1. Ensure stored foods are covered and protected. 2. Ensure in-use scoops are clean and maintained and have handles to prevent food contamination 3. Clean and sanitize the plastic boards to ensure sanitary condition. If unable to clean, replace them. Date to have all corrected by: April 21, 2023 Violation Score: 15 304 - Premises not free of pests [s. 26(a)] Observation: Rodent droppigns were observed in the shelves storing pizza tray pans. Water sources, such as mop buckets filled with dirty water and standing water, was observed on premises which can attract pests. Corrective Action(s): Clean and santize areas with rodent droppings. Ensure standing water and other sources are removed frequently. Block any holes on premise that may be a potential entry point for pests.
